I have got an message on my cic account saying "Medical Report - Section A Client identification and summary (IMM 1017E)"

This is the only message I have and I am confused on what to do?

I need a medical form to make an appointment as well and where can I receive one?

click on the arrow. Arrow should be pointing south when you click it

And then you will see an underlined phrase that says” view this message”. Click that and it will automatically open a pdf file
