IMM 5476 form email from CIC

Hi

I had submitted my Express entry profile last week and have passed my medicals but now CIC has sent me a notification to fill out IMM5476 (using a representative) even though I had not selected one.

How should I proceed now. Have 7 days?

Just write them a letter explaining that you are not using any representative and this seems to be an error.
