Option C conflict

Has anyone that is on a disability generated an Option C (Financial Assessment) form that shows up as Social Assistance instead of Disability. Is this a common misprint - or do I now have to fight with another government office to find out about this mystery?

As long as you have other proofs showing it is social assistance due to a disability, there should be no issue.

It looks like it's going to require me contacting the elusive people that deal with this because the information in question is not indicated on my T5007 tax statement either.
