ITA for 60th Draw

Hello Everyone, Hows PR Application going on.. I started filling the form.. while filling I got some minor issues. please help:

1. In personal Detail Form We have to fill the details of immediate family. Apart from my father no one has surname on passport. so what to fill in the surname column when i fill details of My mother and siblings.

2. I do not have any immediate relative in Canada only my father's distant cousins. Do they count? Guess Not.

3. In personal activity form, I have 3-4 months break between my job since 2012 every year as it was a contractual teaching position starts every year in JULY and ends in either March or April. I usually spend this time vacationing or learning something informally. what should I write for that break period.

4. In Address details, I worked in Chandigarh during 2011 and 2012 for 1 year and 2 months and lived with relatives. but my relatives moved from their place in 2015. so what address should I fill now.

5. my current address is same as my birth address and passport address. so what should be the beginning date of this address my birth year or when i came back to the same address in 2012 after my job in Chandigarh.

please guys help...

Even I am facing same, Last Name issue - My Mother doesn't have her Last Name printed on any document and system doesn't let me enter details if I leave Last Name Blank. Any suggestion ?

Write the first name in the last name box. First name could be left blank.

Hey i tried that filling given name in last name field and leaving the first name blank for each of family memebers but it was an error.

There is something wrong with the website. They mention the same instructions.

hummm.. for the time being i entered their first name in the both fields as NA is also not allowed.. right now collecting documents so will try to change it later.. Maverick if you please look at the other questions in the post...

Once you accept your ITA, your profile frozes so it's of no use again for your point to drop or increase

I am waiting for my FBI PCC and by the time i get this my current IELTS score will be come
invalid so i will have to retake the IELTS exam , had few queries and appreciate if someone can clarify them

1) When do we get to know the document checklist that needs to be submitted.
2) Is Medical test result mandatory before submitting the ITA or that can be done at a later stage.
3) Can i reject the ITA any time before submitting all the documents.

Yes you can reject ITA anytime before it expires.

Once filled all the form in next step the checklist will be shown
About medical m not sure i guess you can do it later but it will slow down the processing.

Medicals need to be done upfront unless there is a genuine reason.

sorry for bothering guys but it seems every time i open my cic account i face a new question or doubt

on my educational documents my father's name has different spelling no surname so ECA do not have it as they assessed my degrees and certificates and added that name only on the ECA. because my father changed his name spelling and added surname officially few years ago. my passport has my father's name with new spelling and surname.

Now should i explain this conflict of name in LOE? will it cause any issue in future?

Also does the old ineligible EE profile counted as previous applications in personal detail form?

I am yet to submit my details and get the Document checklist however i went ahead and booked for my medical appointment , but have some questions around the documents to be taken for the exams
I understand i need to take my passport and 4 photographs and a printout of the ITA letter ,other than that do i need to take anything else ?
How will the doctor tie my result back to my ITA application at CIC as they send the results directly to them ?

Now what information/document do i get after the medical is completed.
Do i get the immigration medical examination (IME) number and a copy of the information printout sheet or the Upfront Medical Report form ?
and i assume one of this detail is required while uploading my documents

When it says First Name(s) it is asking for ‘first and middle name’. I forgot to put in my middle name in my whole profile (EE profile before ITA), however, and it's a locked field now; I can't edit pre-ITA (information provided while creating profile ) form now, in POST ITA form i have update my first name as 'First name middle name' using Modify family information, I can see its updated first name correctly in Post ITA form, But it did not updated in Pre-ITA form . there would be discrepancy in Pre-ITA and POST-ITA form.
will it create any problem? Shall I decline ITA OR Is it OK to provide one and same person affidavit and explain it in LOE
