I had the policy confirmation document but accidentally uploaded the quote. They refused for the following reason: The submitted proof of health insurance does not meet PG-1 requirements, as no proof of payment for the medical insurance coverage was provided.

Keep the dates same and do not forgot to upload the refusal letter and explain in detail that it you mistakenly forgot to upload the confirmation instead of the quote.
