I have received this form...can you please share your experience here...it would be a great help!

I received the same form! My medical is clear on 6th Jan 2021 and I also submitted my Photo and mail address in PR portal but today i got the email (pdf form on my cic account) Can you please help me?

On the letter should an email like :irrcc.mhbSurveillance....
Send your full name, UCI and residential address to that and wait for correspondence from them.

Has anyone received a medical surveillance undertaking form in their MyCIC account?
I submitted my passports and got this in my account. The form is pre-populated and it appears that it is only a declaration on our behalf to allow the government to monitor our health after landing, but there are no instructions on how to send it back to CIC.

Read between the lines of the email. There would be instructions on the email or in the attachment.
