question regarding RTD

1: RTD application has already been submitted, can I re-submit the application? Because I have sent the application to the old address.

2: If yes then fees receipt will be same or I have to pay again.

3:There are three pages for signature. I signed on the first page, two were left. Is it necessary to sign on all three pages?

4:What is their phone number to inquire about the application

Actually I put old address

I had filled the form earlier and it had this old address but later the address changed which I didn't know about.

I am confused about the fee should I apply the same slip or pay again
