Yes, Health Card will be accepted even without a photo. My kid's application was accepted with Her Health Card.
You need to provide address history only for your eligibility period. Out of curiosity, Is your license history from Canada or from another country?
You don't need to provide proof for addresses anyway. I think they just want it on the file just in case they find any red flag on an application and they might want to check the background.
Same for me. Got my AOR on 11th March but nothing moved since then. I regret doing online at times..
Hopefully, this excerpt will solve your doubt. It says provide address during your 5 year eligibility period. So just provide that.

Question 10: Address History
Write all your addresses inside and outside of Canada during your 5-year eligibility period, including the postal codes, starting with your current home address. If you were residing, working or studying outside Canada, you must list all of your foreign addresses, including the country postal or mailing codes. Press the plus (+) button if you need more space. You don’t need to include addresses of family, friends, hotels or resorts where you stayed while on vacation.
Provide information that covers the 5-year eligibility period. Do not leave any gaps during this period and do not leave this section blank. If you do, your application will be returned to you as incomplete.
My wife's and my application were updated recently
In the Old tracker, I saw this update over the weekend:
"We started processing your application on May 1, 2022"
Today, in the new tracker I see the following update:
"Background verification status has been updated to Completed"

Folks, how long does it take for the email to arrive with the instructions to take the online test after the test status is updated in the portal.
It is random. I got mine 2 days after the status was updated on portal.
Did you received the test invite? You can see there is a link on the portal. You can click and go through the instructions there also.

Hmm, it's just May 4th, can you see which day the test invitation will come or something?
