I took a print out and filled only that box with a black inked pen.if you dont have enough space use additional sheet.

How about mothers family name?
In mothers family name at birth in schedule A,

Family name. Luhar

Given name(s). Luhar Bharti Damjibhai (before marriage)
Makwana Bharti Rajesh (after marriage)

Is this what i should write?

I am also having issues with the no. Of characters that we can add in one box.
My school name college name are getting cut short.
Is this fine??
Write the full name in the additional sheet.
Here's what I did, I put reference to attached page in all the cells where I couldn't fit the information like this: "see attached page 5"
Then on the extra page I created the table for the question I was answering and put all the information again. Let's say I had 5 rows in education history so, I added 5 rows on the extra sheet. Hope it makes sense.

Ok.. this is good idea.

@RSub, I read your message. You mentioned that you already had your PCC. Did you apply before hand or did you wait for the ADR request. I am also planning to apply for PCC before hand, as it takes a lot of time. Kindly revert.
If you are not exiting the country of residence any time soon it is a good idea.

But if you are exiting the country for good make sure you get the pcc with a date after you exited.

Finally serving its purpose ;)
So wait in that schedule A form it is written given name( names)
Does given name means first name( usually that is the case)
Or does it require all the aliases

So if it is like what you are saying just use an additional sheet (LOE) to mention the aliases of mom and dad

I am not talking about given names, i am talking about family name given by birth.

In schedule A they have written
Family name given by birth............ and
Given name(s).............. Like this.

In mothers family name at birth in schedule A,

Family name. Luhar

Given name(s). Luhar Bharti Damjibhai (before marriage)
Makwana Bharti Rajesh (after marriage)

Is this what i should write?

Thanks @cansha , I guess this is a lost cause since it has been more than a year :(
I would still suggest to reach out to either your employers immigration department or consult with an immigration lawyer yourself. There may be options. So try a bit more before giving up.

Jokes on you, Russia! Trudeau doesn’t have to visit you to appropriate your culture.
Beaverton
So, you in the Kremlin thought you were safe from cringe-inducing action by Justin Trudeau just because you banned him from the country? Well, unfortunately for you, Canada has been a superpower in terms of socially inappropriate politicians since 1988, and the Prime Minister can inevitably offend your entire nation without ever having set foot on Russian soil.

I mean, just look at the guy. What part of him looks like he’s not ready to pull out a hundred Yakoff Smirnoff bits within seconds of meeting any Eastern European?

Trudeau’s power of cringe doesn’t require any actual presence in your country. All those embarrassing outfits from his India trip? He didn’t buy those there – he brought them with him.

And don’t think you did yourselves any favours either by barring the rest of Parliament either. If anything, you’ve somehow made Trudeau stronger. Rumour has it that the last time the Liberal leader broke out in an impromptu Cossak dance, more than a few MPs joined in.
To be fair, there was very little you could’ve done to avoid an awkward encounter with Trudeau. Even if you pulled out of Ukraine tomorrow, Trudeau would’ve shown up to the next diplomatic meeting in an oversized bearskin hat, demanding everyone do a shot of vodka. If anything, it would’ve just made things worse, since he’d be confident you’d forgive him just like we always do for some reason.
Better brace yourselves, Russia. We have reports he just asked a very confused make-up artist for tips about how he could best pull off “Russianface”.

Is it that complicated  I just wrote my mother's name-surname after marriage. I mean it has been 32 years since they got married. How is her maiden's name is relevant? I am confused, did I miss something?

As per my knowledge
It would be maiden's name for family name by birth.
Because it says FAMILY NAME BY BIRTH.
@legalfalcon please help..
