Hi,

For question 11, where were you physically located ?

Should we enter where our work location was / house address where we were staying when working there.

I work for a consulting company, which provides services for its clients.

Should I mention the location of my client where I was physically present or the location of my employer?

To be frank, this question is ridiculous. If were working for multiple clients during this period, its literally impossible for me to remember the exact months when I was working for each client.

If you want to be super safe - include an explanation letter stating that your physical location changes daily based on your employers clients, however you have provided the address of m your main working office. If they need more details, please contact you...

why not just put the address of the consulting firm since they are your employer. dont know if it makes sense.

You should both put the address of your employer, that is, the consulting company through which you worked. The consulting company was your employer and it is the consulting company that IRCC should contact to verify your employment.

It occurs to me that if you worked onsite for clients that were outside of Canada, then the above advice does not necessarily apply to you. You will want to disclose your work abroad as part of your application so as not to appear as though you are trying to conceal absences from Canada.
