PLEASE HELP- Statutory Questions

Hi Friends,

I am in the process of filling out all Statutory Questions information post ITA. I am a bit little confused to answer the questions.
my application for dependent open work permit visa was rejected twice. For question A, I can answer Yes and give details out. But what should I answer for Question B.
A)Has XXXXXXXXXXXXXbeen refused refugee status, or an immigrant or permanent resident visa (including a Certificat de sélection du Québec (CSQ) or application to the Provincial nominee Program) or visitor or temporary resident visa, to Canada or any other country? (required)
B)Has XXXXXXXXXXXX ever been refused a visa or permit, denied entry or ordered to leave Canada or any other country?: (required)
