Please help - How to get DL Extract from Bangalore, India

Please help me in getting the DL Extract. Currently I am in Bangalore, India. I any one has already been successful in getting DL Extract from Bangalore please help me on following queries:
1. I read online that I can submit form and make payment at any of the Bangalore One branch. Is it true? If yes, whether I would not be required to go to RTO
2. I have DL that was issued 5 years back. However, the car that I drive is not on my name. Will this be any impediment in getting the DL Extract. In fact I have never had any car on my name ever.
3. Please share your experience and process.

Hey, were you able to get extract from banglore? My dl had old address and i am not sure if i should change the address first before giving DL extract. could you get experivcnce inontqrio or they create dfuss about address?
