I have received correspondence from CHC including the following:
1. New Schedule 1 Background/Declaration Form - Note: Corrected reflecting real location of principal & spouse)
2. Medical Forms for Medical Exam
3. RPRF
4. Long-Form Birth Certificate for ---- (Blank Lines)
5. Updated Police Certificate for Principal & Spouse for one of UAE (We lived in there)
6. Last 15 Years of Travel Record for Principal & Spouse
Here are my Questions:
1. Why they asked for CORRECTED Schedule 1 - while I do not have any change or mistakes in the application submitted in Mar, 09.
4. What is Long-Form Birth Certificate? - We might not have them, they did not mention any name. When myself and spouse were born, it was used to be midwives executing these at homes. Might have Kids discharge certificates from hospitals... is that what they need?
5. Why they ask for PCC from UAE while we live in another GCC country since 5 years?
Please advise?
