LMIA extension

Hi, I was wondering if an employer's LMIA has reached its 18 month effective period is there a process of extension, or do they need to restart the entire process?
Then as a worker can I continue working, or do I need to stop working, or do I need to leave the country while that is pending?
I currently hold a valid 2 year employer specific LMIA work permit.

For an employee of mine, we did another LMIA as their work permit was expiring end of last year. They never had to leave the country.
