imm 1017 - HELP!!!

Hi everyone,

With the new website for CIC all changed around I can't seem the find the medical form that my hubby needs for the DR to sign. It was there the other day and now it is gone....not even on the drop down where all the forms are.

Does anyone happen to have a copy that they can send me if I give you my email? He has an appointment in 2 weeks for the DR and I need to get this down to Jamaica for him

Any help would be great....I've already had to redo some of the forms because of the changes to them

Coinciding with the implementation of eMedical, will be the release of GCMS 4.0 which will include new medical screens. As of November 1, 2012 the new medical examination forms will be disseminated to all panel physicians.

With the release of the new GCMS 4.0, it should be noted that all previous versions of the IMM 1017 form as well as the Appendix C will no longer to be used. With this release, there will be two newly-revised IMM 1017 forms:

IMM 1017: Medical Report: Client Biodata and Summary (not available on our website as it will be GCMS-generated along with medical instructions)
IMM 1017B: Upfront Medical Report: Client Biodata and Summary (not available on our website as panel physicians will have these forms in their office. With the implementation of the new IMM 1017B, Appendix C will no longer be required)

So my interpretation (please correct if others have other experiences!) is the only thing you need to bring to the doctors is your photos, and all the required docs will be available at the doctors and also filled out there. CIC site sure makes it confusing.

I had the same panic moment. URLs were broken, forms weren't loading... I thought I was going crazy trying to locate Appendix C and IMM 1017. Called immigration this morning and they didn't even update me on this info, the woman said "you can bring either Appendix C or IMM 1017 to the medical exam, they're the same thing."

Just to make sure, i would definitely call the doctors office first before you go.

If the CIC can't even be bothered to update their own forms on the website, nor all their CSRs on the help line... i also doubt they have properly informed all the doctor offices in the world that are authorized to do the immigration exams. So would not be surprised if some of them are still expecting you to bring in all the old paperwork. I would still print out and bring the Appendix C form to be safe (available here: http://www.cic.gc.ca/english/resources/publications/dmp-handbook/pdf/AppendixC_e.pdf )

I have saved the form and emailed it off to my hubby. Told him to print it off and bring it to the Dr's just incase it's needed
