If your total duration of stay in India in the past 11 years is more than 6 months, then you need to provide it. If it is less than 6 months, the you don't. Either case, to be on the safe side, I'd recommend you to go for it.

But, what about the dates for Country of Origin? Should I apply until my last visit which is just one month during Masters or until the first time I left the country for Masters?
