OFW po kami, my question is how to fill up the form IMM5409E or Statutory declaration of common-law union? Sa part po ng Solemn Declaration, ano po or sino po ang ilalagay sa "Name of the person who administered the declaration". Do we to go to the embassy to fill this up?

Kapatid, salamat sa reply mo. OFW kase kami ng mister ko, wala kami sa Pilipinas. Sa form na yun may nakalagay na "Name of the person who administered the declaration" saka signature nya. Any idea kung sino yun? Do we need a lawyer? Naguguluhan kase kami o_O:(
