oh im not sure. because mine is the same way the pic is in front then at the back there is a print and i included that in my application.

hi! you should check the back part of that page because it is printed back to back. well that is mine looks like. it has 1 page but has a print back to back. at the 2nd page you will see the "consent" hope that helps. :)
