In my request letter it states, please provide photocopy of your passport (all pages).... So I scanned all stamped and blank pages but I didn't include work permits from Canada.

Re: Mentioning work history in Personal Details DO NOT claim your history in that field if you don't have reference letters. Leave it blank and just claim Canadian work history.
