I'd say either 2171 or 2173.

if you have a supervisa, then yes max term of single stay is 2 years. However, in my case, I didnt apply for supervisa. I simply applied for visitor extension for 2 years, however I did add one year medical insurance with the application to support the request.

I think it will be the date of medicals of the primary applicant, not 100% sure about it though.

Carry the PDF print attached in the PPR email. Fill the Bio Information pages by hand. Carry two photocopies of passport bio pages. Most VFS have got instant photo click facility available, check the VFS website for details.
