Yes, I did. On each sheet, indicate name of the form, your name or your spouse . At the bottom naman, affix signature and indicate the date. I also had a cover letter indicating that I have attached additional sheets

Ok lang if mag overlap since yun naman talaga ang actual scenario, to be safe you may attach a letter of explanation just like what I did. Then when it comes to dates, start with the current/ latest. Hope it helps... Good luck! :D
