You should better attach FRC (Family registration certificate) from NADRA. It is record of all your family members with date of births. It will cost you around Rs. 1,000 and same day process

You have to send back your passport obviously. Medical reports will be sent my Medical centre themself. just take a medical receipt, send a copy of it with your passport
