thanks for update sponsorparents, below in form only space says signature nothing specified for Applicant or Spouse. If i fill seperate forms for both than do have to put Principal applicant area my mom's name instead of my dad's name or not? Thanks

Just update, parents went to Apollo Hospital today and you are right they Did not asked for Medical forms and given appointment for Friday morning for Medical examination. Hope everything go smooth without any more troubles. one question - do i have to fill seperate form for parents...
