It should be fine as long as you carry your marriage certificate to explain the name change at the border.

I would just write an affidavit where you give a statement of facts about your case, explaining why the names don't match.

It's not super clear, are you saying your parents and you don't share the same family name? Even then, I don't think it'll be an issue (people change their names when they get married, for example). But you could add your birth certificate or family record book to your visa request if you want...
