When I applied for PR years ago I was living in an other country and I had an expired canadian visa in my passeport. It was so complicated to mail my passeport from my country so I gave it to my sister, she posted it from Canada and had it returned to Canada.

For the adress there is no problem. I am sure. for the others points I can't help you.
