X-ray after delivery

Hi All,

I need your prompt assistance on the following:

I delayed the x-ray part of my medicals for my PR application after delivery and the baby will be born in Ontario province (where we claimed to reside in our PR application).

As far as I understood I need to submit the following documents:

• Doctor's form on the x-ray completion
  
• Birth certificate of the baby along with the hospital's proof of birth documents
  
• IMM 5406 - Additional Family information form
  
• A letter of explanation

Which type of Ontario birth certificate is suitable for the PR application? And what additional documents do I need to submit when adding a newborn into the PR application? Do I need to pay any extra fees?
