wrong address in form

I applied for a work permit around a month ago from outside Canada and just realized I put the wrong residential address in one of the forms.

I had a medical exam, and my doctor was the one who noticed the mistake so when he submitted the results, he put the right address.

How can I fix it on the form/application?

I know the web form, but I can't find a link, if anyone could provide it.
