I applied from outside Canada and am in Canada with wife and child on a visitors visa. Sponsorship has gone through, waiting on medical/interview/PR approval. I need to work to support my family.
What is the best way to get a work permit without a pre-existing job offer?

Also, since I applied from outside Canada, can I get my medical done in Canada?
 
Just to be clear - did your wife sponsor you through the outland process?

If so, then you were actually supposed to complete the medical before submitting the application and were to include proof of the medical along with the application. I would visit a DMP in Canada as soon as possible and get the medical done (you can certainly do the medical in Canada). Once you have your file number from CIC, mail the proof of medical in along with your file number.

If you applied outland, then unfortunately that does not qualify you to apply for an open work permit. If you want to work before you are approved as a PR and land, then you will need to find an employer first who is willing to hire you. That employer will then have to submit a Labour Market Opinion for approval. If the Labour Market Opinion is approved, you can then apply for a Closed Work Permit.
