Work permit extension

Hi ,
i am filling my work permit extension form and #3 on the IMM5710E form asks what category am i applying for
i am not sure if i have to fill "A work permit with the same employer " or "Restoration of your status as a worker "
My work permit is valid till November 2014 and i am on a closed work permit with the same employer

Please clarify .
