About 9 months ago I took a Immigration Medical Exam. Right now I need to extend my work permit, however, when I try to apply one of the requests is a document for Medical exam. Unfortunately. I was never presented with any documents from the hospital and was told that they would be sent to IRCC. I'm not sure how to fill out the document section for this.
