While going for Medicals documents to Carry???

Hi Seniors ,

I would like to know that while going for Medicals what all documents we have to carry apart from Medical letters received and Passports for all applicants.

Also will all Passports old and new have to carry???

And I would be taking appoitment from Rajiv Khosla for 8th May. So shoudl I take appointment immediately for that date or can I take same in May also.

Our appointment is next week. They asked us to get following with us:

- Original current passports for each applicant ( the ones you sent a copy to CHC with your application)
- Passport photos for each applicant (atleast 2)
- If you wear prescription spectacles - please carry them with you.
- Any prescription / chronic medication - take the names / letter from your general family doctor.
- Medical forms with your photos on them as sent to you from CHC
- Any other operations/medicals problems - carry letters from your doctors to say it is clear.
- Fees as per the doctor.

If the doctor's office allows you to make an appointment now for the 8th of May - why dont you go ahead! Atleast you have peace of mind that you have the appointment!

Thanks a lot. I have already taken the appointment for 1st May.

Yes they have asked same. But they wanted to have Passport photocopy of first and last page on single page for all applicants, plus other details taht u have mentioned above..

Yes few DMPs ask for Passport Photocopies too..
