I studied abroad in Israel for over 6 months, so I need to get a police certificate. The CIC website notes that the Israeli government won't give the certificate to private individuals and that "Police certificates may be sent to either the Canadian Embassy in Tel Aviv or to the Israeli Embassy or consulate abroad." Does anyone know what addresses would be associated with those places? Has anyone had prior experience with where to send it? Should I include by EE profile number somewhere?

Check with Israel consulate website at your country of residence for information.. i guess they forward the certificate directly to CIC.

Consulate General of Israel in Toronto - have below info..
http://embassies.gov.il/toronto/ConsularServices/Pages/Police-Clearance.aspx

You may apply in person at the Consulate. You must make an appointment on our website, complete the appropriate form and bring your passport for identification. Please come prepared with the address of the office requesting a police clearance, a file number (if applicable) and a registered mail envelope so that we will be able to forward the certificate to the requesting office. If you do not bring a registered mail envelope with you, you will be charged $15 for one.
