Where do I upload the Schedule 4 form?

I just received a letter from CIC asking me to upload it within seven days. This is fine and great! I can do that! But

a) Where do I find Schedule 4?
b) Where do I upload it to my profile? I can't find any place in the profile to do so...

Okay, yeah, it's apparently in the submitted documents checklist.
Weird.

Also, does this need to be physically signed by me? The reason I ask is because it is an ePDF, and I can fill it out and upload it directly without issue. If it DOES need me to physically sign it, then I guess I will print it out, sign it, then scan it and upload it.
