When submitting package, include this page at the top . ..?

Dear All

The letter I received from VO says that I have to "include this page" (first page) of appendix 1 at the top of my documents package . . .

But this page is only the first of several subsequent pages that list all the documents required

So Shall i include this page only at the top or the entire appendix 1.

Also . . instead, Cant i just make my own cover note and mention the list of documents that I am sending along with notes and elaborations

Do as they say: In my case, I included that page that has the case number and the Payment Received No. But... I also, sent a COVER LETTER WITH THE REF.: Case number and Name. Describing all the information in the package very organized.

It worked fine for me....
