does it strictly mean they are doubting the spousal relationship to be genuine? Or it could be related to other aspects in the application?
There is no indication in the GCMS notes why review is required, and nothing in the Officers notes highlighting any concerns.
We are trying to understand what does this relate to and maybe send more proof of relationship (communication, time spent together, etc) in order to avoid more delays or a possible interview. Any idea what does this status usually mean plz?!

It doesn't mean anything, it's just a term they use. If you need an interview you will see it at the end where it says "Notes"

That's why I'm concerned and asking if it is strictly related to the relationship being genuine or it could be something else.

Eligibility will cover all aspects of eligibility.

Your question is quite inexact, so honestly don't understand what you're looking for.

The GCMS notes are quite long so two words "Review required" out of context doesn't say much. Which aspects of eligibility are looked at depends on stage and section in which the words were found.
