Whats the procedure to extend a study permit

i want my niece to extend her study permit. i was wondering what the steps are.
i read that you need to do a medical exam. is that after you submit the study permit extension.

You Pr niece will have to show why she needs to extend her study permit and that she has the funds to pay for her more studies and living expenses.
