We are FINALLY sending our app in this coming week. The only thing missing is the letter from my employer stating how long I've worked for the company and my salary. I'm going to write the letter myself and have my boss sign it on Monday.

What, precisely, needs to be in the letter? Who is it addressed to (To Whom It May Concern or something else??)? I assume it should also be printed on company letterhead as well... do I need to include a return address/contact information for my boss?

It may be addressed to "To Whom It May Concern", that is fine.

It should state your full name, your position, when you assumed the position and a brief description of your responsibilities/duties, your hours of work and your rate of pay, whether per hour or annual.

It should definitely be on company letterhead, and include daytime contact information for your employer. Your employer's full name and position should be stated, and if you are in their direct line of supervision or however they interact with you should be stated.
