What should I ask the doctor to give me when I have my upfront medical?

Hello Everyone,

I am doing my up front medical tomorrow, I just want to be extra careful so I wonder what kind of proof/document I should ask the doctor/office to provide so that I can use it as a proof that I have done my up front medical in my e-APR?

well, in my case I've called in to book an appointment, then i told them what I want to do which is an upfront medical exam for Express entry and then they did all by themselves.

I only have a signed agreement with the doctor with my details... nothing else.
