What pages to scan from Passport

I assume you are talking about the I-94 slip that US border guys staple to your passport. If yes, No you don't need to scan that. Just scan the pages on your passport. (BTW it doesn't any negative effect on scanning it and including. In my case I did scan and attached it but it's not necessary)
