WHAT MEDICAL FORM TO TAKE - PLEASE HELP

Hi

I am going to my medical and I am not sure which form to take.
I am applying out-land as a spouse. Do I just need to take section
C of the region specific form?

Does the DMP provide me with the copy of the Medical Report: IMM 1017 EFC - Medical Report Section A?
Or do I need to take this form?

Also, do I include the appendix C in my application or does the DMP keep it?

Yes you should take Appendix C of the region specific form with you and they will keep that form. They will provide you Copy 2 of the Medical Report-Section A that the DMP will sign and give to you when you undergo the medical examination.
