What is the "Employer Questionnaire and Declaration"

I was trying to applying workpermit from LMIA.
I applied since last year on september, but they are doing on review our document. However, our workpermit gonna expire Feb. 06.
That's why we will try to apply work permit, but we know it will be reject. But we need to staying here so that term we can work until we are getting the reject letter.

But the thing is I check the what I need documents, it was something weird.
I can prepare everthing from find Q&A except "Employer Questionnaire and Declaration"this documents.
if anyone know about this, could you plz submit about this info, or what I can prepare from by Employer?

Hi!

We came up with the same problem, and it became a headache for more than three hours, there was no information anywhere! At the end we decided to review the questions of "check your eligibility" from the beginning and realized that we had answer that "yes, we had a declaration form from our employer", which we did not, so we changed the answer to "No" and the "employer questionnaire and declaration" disappeared from our application! Then we could pay our fees and submitted it!
