What forms?

Just wanting to double check on some of the different forms I need to include. Our appliction is relatively basic, my spouse is Canadian and sponsoring me through marriage, no children or dependants etc involved.

That being said do i need to fill out IMM 5406 additional family information? Can someone who has done it recently give a list of the forms that they included? Thanks

Yes, you (the applicant) needs to fill out the top part (Section A) and the bottom if you have any siblings (Section C).

Ok thanks, it says on the form that the principal applicant and 'their spouse' should fill out their own copy of the form. So we have to include one for me and one filled out by my spouse?
