what form to fill out for open work permit

hi guys.i hope someone can help me here.. we are done sorting out our paper and its all ready to send but I found out that I can also send an application for an open work permit along with the application package but my problem is that there is no form for open work permit..do I just use the regular work permit form,, and when I open the form in adobe its not giving me any option as well if I am applying for an open work permit,,the choices are just if you are extending your stay with the same employer or new employer ,restoration of status and if its initial temporary RESIDENT. what should I choose with these choices guys or is there a different form that I should use and where do I get it.
