Hello,
We are requesting deporation documents from the USA. Finally, we have found the correct form to do so (G-639, Freedom of Information/Privacy act request)

On the form under 'verification of identity' it says: "...The request must be signed and the signature must either be notarized or submitted under penalty of perjury."
So does this mean, My husband can sign his name and just write beside his signature signed under penalty of perjury. ?????
Just want to make sure and save some $$$ if we dont have to get notarized
