Hello,

We are requesting deporation documents from the USA. Finally, we have found the correct form to do so (G-639, Freedom of Information/Privacy act request) :P ;D

On the form under 'verification of identity' it says: "...The request must be signed and the signature must either be notarized or submitted under penalty of perjury."

So does this mean, My husband can sign his name and just write beside his signature signed under penalty of perjury. ?????

Just want to make sure and save some $$$ if we dont have to get notarized :)

No, it MUST be signed by a notary. And what it means is that by signing it and having it notarized you are guaranteeing that it is, in fact, the person signing the form as is named in the form. If you lie and say you are the person when in fact you are not, you have admittedly committed a criminal offense. Of course, this does not apply in the case of a minor child where he/she would need to have a parent/guardian signature.
