WES Academic Records Request Form

Hi Guys,

I'm filling the WES Academic Records Request Form and I have some questions about the section B For Authorized Officials.

The University wasn't sure what to write in the field " Title " ( if the title was for the student or for the professor)
And also what do fill in the field " Degree Obtained " ???
Thats why they told me to ask someone who knows .
