I've been in a relationship with my girlfriend (who currently lives in the UK, whereas I'm a Canadian) for nearly four years now. We've been visiting each other back and forth a couple of times a year, but its getting to be too much, and we miss each other terribly in between visits.
We'd been hoping for her to get in through the International Experience, but she didn't make the cut.
This was quite depressing for both of us, and now with our immediate plan having failed, we aren't sure what to do. We're both twenty-four, and while I work full time, she only works a few times a week due to illness. She wants to move here with me more than anything, but we doubt very much that she'd get in on a work visa.
We were thinking perhaps Marriage, but I can't find any actual solid information on where exactly that would put us. Would her marrying me here give her permanent residency? Would she be able to work, or would she have to return home? Is there a chance she could be denied PR for any reason?

I would recommend that you start by reading the first post in the following thread:

http://www.canadavisa.com/canada-immigration-discussion-board/spousal-sponsorship-t46995.0.html

Marrying you will not give her PR - but you will be able to sponsor her for PR once the two of you are married. Marrying you will not allow her to work automatically.

Again, read the first post in the thread above to get you started.

We were in your shoes back then. I'm sorry to hear she wasn't able to get the IEC. I guess it was gone in minutes? I know how hard it is to keep a long distance relationship. Like Scylla said I'd give it a look on that link. Or live together for a consecutive period of 12 months together proving shared rent, account and things like that to be able to sponsor her. But have a look at that link and see what you'd find.

If she is ill how is she going to pass the medicals?

The medical restrictions are very low for a person being sponsored as a spouse. Aside from TB, or anything that could be deemed as a real risk to others, most conditions are a non-issue.
