Your wife will need a Canadian License if she wants to practice nursing in Canada which is difficult to obtain with a foreign degree. Also LMIA is applied from the employer so if she wants to get LMIA, she will have to look for a employer that will provide a job offer and apply for LMIA. This will take 4 to 6 months processing with no guarantee that LMIA will be approved.
She can certainly apply for a work permit as a nanny if she can find an employer who is willing to give her a job offer and obtain an approved LMIA. However that won't allow you to remain in Canada or qualify for an open work permit. The job has to be NOC A, B or 0 in order for you to qualify for an open work permit and be able to remain in Canada with her.
She will have to find a employer first to get LMIA. Whether the employer will help her obtain LMIA is up to them because LMIA is very difficult to obtain and the employer will have to spend a lot of money in order for her to work legally.
This is not a fast process. You are looking at 4-6 months if you find a job and employer willing to go through the time and expense of a LMIA. Then you need the LMIA approval. You then will have to apply for a work visa.
