Visitor visa extension for parents visiting kids

Hi all,
I need help to apply my mom's visitor visa extension. She is now on 10 years multiple entries traveler visa which has only up to 6 months. Now I'd like to extend her study to over 6 months because she's looking after me while I'm doing my secondary study in Canada. I check the CIC questionnaire. The document lists only has the following :

• IMM5708
• Passport
• proof of means of financial support
• digital photo
• family member proof of status

however, my friends told me I also need to submit my birth certificate and school attendance letter etc. These seems not required on the document list.
Does anyone know do I on the right page for the application? I think I need to apply for a paper called" visitor record" for my mom.

The CIC questionnaire is correct. Those are the documents needed.
