Visit visa rejected on past refusal when never applied before

Hello, my sister visa is rejected and gcms notes says she hid her last refusal when she never rejected before. They asked for last 10 yr travel to canada and she had come once before 15 years ago so she said no on last 10 years. Could that be reason ? And also that was also not a rejection she actually visited canada. Also gcms notes have date from 2017 and 2019, what does that mean

Did she get a visa refusal somewhere in the world not just Canada? Could a family member have applied on her behalf?

No refusal anywhere in the world for those dates. She applied Canadian immigration somewhere in 2009, but the application was returned, she didn’t pay any fee or anything for that and she doesn’t have any recollection of why was it returned or her file number. How can we obtain info on that

You can apply for reconsideration explaining that they have no recollection of that visa application and you would like them to verify that an error hasn’t been made.

So you did misrepresent yourself. If only reason for refusal, reapply and don’t include all refusals.
