Visas are valid from;

1. Date of appearance for medicals at IOM / Panel Doctor - As medicals are valid for one year
2. or it expires earlier , if passport expiry is earlier than Medical Expiry

and on average after how many months of medical would one get permit . I assume on average one is given at least 6 months to prepare things and land in canada .
